If you're the parent or guardian of a child aged 18 and under, use these forms to put their new name on public record by 'enrolling' it at the Royal Courts of Justice. Includes guidance note (for minors).

If you are over 18 and want to enrol a change of name for yourself, you must use the guidance and forms for changing your name by deed poll. You cannot enrol a change of name yourself if you’re under 18 years old.

Open a document

  3. Save the form (in your ‘documents’ folder, for example).
  4. Open Adobe Reader and then select the saved form.

Microsoft Word
